    The COVID-19 crisis has created a watershed moment for the NHS, demanding a reappraisal of how essential services are delivered to the public. Even prior to COVID-19, the NHS recognised a pressing need to rethink healthcare using user-centred design principles, based on populations, not organisations. With the advent of the pandemic that pressing need has become an operational imperative. Digital capability has been and will continue to be a key part of transformation, but will only work when aligned with reforms in other key enablers such as financial flow, workforce planning and regulation. Many industries have already made the shift to enabling collaboration and innovation through more agile models of delivery by embracing technologies like artificial intelligence (AI), internet of things (IoT) and/or flexible and secure forms of (multi) cloud storage. Health, on the other hand, until now has introduced new technologies with the objective of improving existing pathways and service delivery models. There is now an opportunity to reimagine healthcare, driving true transformation enabled by digital capabilities.

    As the NHS recovers from the first wave of the COVID-19 crisis, many organisations and health systems are not seeking to return to their pre-Covid ways of working. Instead, they are using the ‘reset and recovery’ phase as an opportunity to transform and enhance patient care whilst locking-in efficiencies and operational improvements. This transformation is seen as essential by many as the NHS prepares for ‘Winter Pressures’, builds resilience for any future Covid waves and, importantly, manages the backlog of elective procedures. The pace and extent of disruptive transformation driven by the Covid crisis would have been unimaginable at the turn of the year. Since the pandemic erupted, organisations and networks across the NHS have implemented, almost overnight, many transformation initiatives that have been in planning stages for months or years. It has also necessitated a radical redesign of many ways of working. These changes have led to a fundamental rethink of both the speed and level of change that is possible. Despite all of these pressures it is recognised that the speed and level of change must be implemented in a managed and phased way. This webinar will highlight: The challenges the NHS faces.     Sarah Seddon's son (Thomas) was stillborn in May 2017. The lack of candour following Thomas’ death and the conduct of the serious incident investigation impacted significantly on Sarah and her family. The local investigation was followed by a Fitness to Practise (FtP) investigation where Sarah experienced how damaging, dehumanising and traumatic FtP processes can be for patients who are required to be witnesses.     Sarah Scobie, Deputy Director of Research at the Nuffield Trust, looks at the continued high numbers of people dying at home, even as hospital deaths return to close-to-average levels, and discusses what reasons might lie behind the continued high numbers of home deaths since the onset of the pandemic. Whatever the reasons for the greater number of deaths at home, a third more people are now dying at home than prior to the pandemic. Although it is widely thought that many people prefer to die at home, this shift presents a significant challenge for community health and care services to deliver high quality care for patients, and to support families at the end of life.

    This checklist is the first step in identifying and prioritizing areas of action for improving the protection of health and safety of health workers in line with WHO–ILO Global Framework for National Occupational Health Programmes for Health Workers. It is designed to be filled out in discussion with management, responsible officers for occupational health, environmental health, infection prevention and control, human resources and representatives of workers in the health facility. This participatory approach will provide a variety of perspectives and a more comprehensive basis for identifying the existing preventive measures, possible problems and solutions for continuous improvement. Using this checklist to begin this process will give you an overview of areas where you can propose to take action and help you determine what to prioritize. These priorities guide you in planning for improvements.

    Myalgic encephalomyelitis (ME)/chronic fatigue syndrome (CFS) is characterised by persistent and disabling fatigue, exercise intolerance, cognitive difficulty, and musculoskeletal/joint pain. Post-exertional malaise is a worsening of these symptoms after a physical or mental exertion and is considered a central feature of the illness. Scant observations in the available literature provide qualitative assessments of post-exertional malaise in patients with myalgic encephalomyelitis/chronic fatigue syndrome. To enhance our understanding, Stussman et al. formed focus groups and listened to patients’ experiences to better understand post-extertional malaise. The authors found that the experience of post-exertional malaise in ME/CFS varies greatly between individuals and leads to a diminished quality of life. ME/CFS patients describe post-exertional malaise as all-encompassing with symptoms affecting every part of the body, difficult to predict or manage, and requiring complete bedrest to fully or partially recover. Given the extensive variability in patients, further research identifying subtypes of post-exertional malaise could lead to better targeted therapeutic options.

    The COVID-19 pandemic is a traumatic event for many, particularly those in the caring professions. Experts are predicting a significant “second curve” of mental health problems among both healthcare workers and the public related to prolonged social isolation, loss of economic opportunity, grief from losing loved ones, among other causes. While there has been no shortage of resources and recommendations designed to help healthcare workers manage stress during the pandemic, there’s a tendency to place the burden on the individual. At a minimum, it is important that remedies acknowledge the shared responsibility of the healthcare system for creating the conditions for fear, anxiety, and burnout in the first place. In an effort to streamline, provide sensemaking, and support care teams during this critical time, the Institute for Healthcare Improvement (IHI) synthesised themes from several key publications, expert interviews, and five years of experience gained by partnering with health systems around the world to address staff well-being and joy in work. Three key areas to focus on have emerged, along with specific actions healthcare leaders can take to support their workforce and address the more immediate and longer-term effects of the pandemic.

    When crises happen, staff health and social services rise to the challenge. No-one knew exactly what the impact of the new coronavirus, COVID-19, would be but it was clear that everyone would be required to adopt new and different ways of working. Here is the story behind Healthcare Improvement Scotland's new National Wellbeing Hub website which is part of a network of support provided for all health and social care staff wherever they work in Scotland, and unpaid carers, that helps people look after themselves. The Hub complements other national support initiatives such as the National Helpline, and those provided at local level by NHS boards, health and social care partnerships and local authorities.

    Chris Maddocks has dementia and on 28 July, after suddenly becoming unwell, she was admitted to her local hospital. She shares her experience of being in hospital and explains how small things can become much bigger for someone living with dementia. She hopes by sharing that this will help others who may be admitted in the future.

Steven Shorrock asked a variety of practitioners to give a short answer – whatever came to mind. The themes that emerge centre around people, their activities, their contexts, and their tools. Many insights concerned the varieties of human work, goal conflicts, design, training, communication, teamwork, social capital, leadership, organisational hierarchy, problem solving and innovation, and – generally – change. Steven Shorrock is an interdisciplinary humanistic, systems and design practitioner interested in human work from multiple perspectives.
