    Harold Shipman was an English doctor who killed approximately 15 patients while working as a junior hospital doctor in the 1970s, and another 235 or so when working subsequently as a general practitioner. Is it possible to learn general lessons to improve patient safety from such extraordinary events? In this paper, published in the US Journal of the Royal Society of Medicine, it is argued that it is not possible fully to understand how Shipman came to be such a successful and prolific serial killer, nor to learn how the safety of healthcare systems can be improved, unless his diabolical activities are studied using approaches developed to investigate patient safety.

    Internationally, safety of care in child and adolescent mental health has received limited attention. Attempts to understand this area have mainly focused on issues of safety in relation to safeguarding on the one hand, or lack of access to services on the other. There is a call for clinicians, service developers and researchers to consider harm and safety more generally in child and adolescent mental health service (CAMHS). America and other countries have begun to initiate discussions on the possibility of harm caused by psychotherapy, however, the lack of shared definitions as to what constitutes safety and harm present ongoing challenges. To start to rise to these challenges this paper, published in Current Treatment Options in Pediatrics, outlines a possible framework for considering harm in relation to child and adolescent mental health provision.

    Due to the high morbidity and disability level among diabetes patients in nursing homes, the conditions for caregivers are exceedingly complex and challenging. The patient safety culture in nursing homes should be evaluated in order to improve patient safety and the quality of care. Thus, the aim of this study was to examine the perceptions of patient safety culture of nursing personnel in nursing homes, and its associations with the participants’ (i) profession, (ii) education, (iii) specific knowledge related to their own residents with diabetes, and (iv) familiarity with clinical diabetes guidelines for older people.

The app includes a seizure video function, symptom log and diary to help keep track of seizures and aid diagnosis. It also features key emergency and contact details, an information library tailored for either adults or young people, and provides data in both email and chart format that can be easily shared with a school, carer or medical professional.

    The 2013 Child Health Review into Epilepsy highlighted the importance of clear and comprehensive care plans for parents, schools and others caring for children and young people with epilepsy; providing them with information on how to respond to prolonged seizures. This finding supports the recommendations on emergency care plans as set out in the National Institute of Clinical Excellence (NICE) and Scottish Intercollegiate Guidelines Network (SIGN) guidelines. A key recommendation from the review was for clinical teams looking after children and young people with epilepsy to consider introducing an 'epilepsy passport' as a means of improving communication and clarity around ongoing management.

    In recent years, there has been an increasing focus on the role of safety culture in preventing incidents such as medication errors and falls. However, research and developments in safety culture has predominantly taken place in hospital settings, with relatively less attention given to establishing a safety culture in care homes. Despite safety culture being accepted as an important quality indicator across all health and social care settings, the understanding of culture within social care settings remains far less developed than within hospitals. It is therefore important that the existing evidence base is gathered and reviewed in order to understand safety culture in care homes.

    Thank you for the comment @Andrew Ottaway I saw that the FTSU guardian commented on twitter that the private sector have to have a FTSU guardian if they are providing NHS care (in this case I don't think they are) So what happens here? I expect people using private healthcare do not realise how unsafe their care could be. They are paying the price - literally. Terrifying

    Medicines reconciliation and medication reviews play an integral part in medicine optimisation. Medicines reconciliation is the process of accurately listing a person’s medicines. This could be when they're admitted into a service or when their treatment changes. It involves recording a current list of medicines, including over-the-counter and complementary medicines. Then, the list is compared with the medicines the person is actually using. It involves recognising and resolving any discrepancies and documenting any changes. The medicines reconciliation process will vary depending on the care setting that the person has moved into (or from). Trained and competent staff should carry out the medicines reconciliation. They should consult with a health professional. Ideally, this should be the person’s GP, nurse or pharmacist.

    Following the posting of the recent anonymous blog by a brave nurse - a discussion was started on Twitter about the aspect of accountability, duty of candour mixed with a no blame culture. If there has been a drug error: The person who did the error needs to feel secure in the knowledge that there is a no blame culture, otherwise they may not report it in the first place. The patient needs to be told that they has been an error with their care The person who did the error needs to be held to account So, can these three points coexist or are we wanting the impossible?

    The Montgomery case in 2015 was a landmark for informed consent in the UK. Nadine Montgomery, a diabetic woman and of small stature, delivered her son vaginally; her son experienced complications owing to shoulder dystocia, resulting in hypoxic insult with consequent cerebral palsy. Her obstetrician had not disclosed the increased risk of this complication in vaginal delivery, despite Montgomery asking if the baby's size was a potential problem. Montgomery sued for negligence, arguing that, if she had known of the increased risk, she would have requested a caesarean section The Supreme Court of the UK announced judgement in her favour in March 2015. It established that, rather than being a matter for clinical judgment to be assessed by professional medical opinion, a patient should be told whatever they want to know, not what the doctor thinks they should be told. This ruling means that patients can expect a more active and informed role in treatment decisions, with a corresponding shift in emphasis on various values, including autonomy, in medical ethics
