    David Cousins, a consultant in safe medication practice, discusses in a correspondence to The Pharmaceutical Journal how the NHS may have forgotten its safety memory in not addressing important medication risks. Since the introduction of the NHS Patient Safety Strategy for England in July 2019, Cousins states that the NHS is not transparent enough to identify medication risks and calls for safeguards to be put in place.

It is FTSU month in October and I was wondering if anyone had used the service, would they like to answer a few questions. We can post this on the hub, so people can see how the system works and how it felt to raise concerns. This of course would be dealt with strict anonymity, as these issues may be sensitive.     By posting anonymously speaks volumes to me. It is a reflection of the fear that is entrenched in speaking up in healthcare. Fear of putting your head above the parapet and fear of vilification. I wonder if their Trust knows that this is even a problem?
