Content ArticleWhen people already negatively affected by unfavourable social determinants of health seek care, healthcare itself may make health inequalities worse, rather than tackling them. This is seen in certain demographic groups experiencing disproportionate levels of harm. This article in The BMJ argues that focusing on patient safety in terms of specific health inequalities will help make healthcare more equally safe. It looks at interpersonal and structural factors that shape care experiences for people from marginalised backgrounds, including poor communication, basing treatment on models built around majority norms and healthcare worker bias. It highlights the importance of having a clear line of accountability for unequal harms so that individuals and organisations are given responsibility for taking action to overcome issues.

Although examined in other industries, little has been done to examine how incivility impacts the ability to deliver safe care in a crisis. Katz et al. sought to determine how incivility influenced anaesthesiology resident performance during a standardised simulation scenario of occult haemorrhage.

Of the two, psychologists say it’s the narrative that matters most. Creating coherent stories about events allows us to make sense of them. It is the narrative that determines our reactions, and what we do next. Two years after the World Health Organization (WHO) finally used the word “pandemic” in its own story about the deadly new virus from Wuhan, narratives have multiplied and changed around the big questions. How bad is it? What should we do about it? When will it be over? The stories we embraced have sometimes been correct, but others have sown division, even caused needless deaths. Those stories aren’t finished – and neither is the pandemic. As we navigate what could be – if we are lucky – Covid’s transition to a present but manageable disease, it is these narratives we most need to understand and reconcile. What has really happened since 2020? And how does it still affect us now?

Crossword counterpoint: glimpses of NHS whistleblowing terrain

This blog is prompted by a recent newspaper crossword in which one of the clues, quadruplicated, was 'Whistle-blower'. The four answers were, respectively, 'canary', 'snitch', 'telltale' and 'betrayer'. The blog draws attention to negative perceptions of whistleblowers in the eyes of some people. It emphasises how wrong these perceptions are and how damaging this can be, with serious patient safety implications. In this blog I provide a crossword counterpoint (attached below to solve), which seeks to support learning about the realities of hostility against some staff who speak up in the NHS. I will share a follow-up blog which contains the solution to this crossword and seeks to provide further education on this topic where there is so much confusion and misunderstanding.- Posted

Content ArticleThis investigation by the Healthcare Safety Investigation Branch (HSIB) looks at the issue of emergency blood transfusions given to newborn babies who need resuscitation when they are born. If a baby has lost blood before or during birth, efforts to resuscitate them may be less effective because they may not have enough blood to carry the oxygen their body needs. Delays in the administration of a blood transfusion in this scenario can therefore result in brain injury caused by lack of oxygen to the baby’s brain. As its ‘reference case’, the investigation examined the experience of Alex and Robert, whose baby Aria was born by emergency caesarean section following an acute blood loss. Baby Aria required resuscitation and was given a blood transfusion before being transferred to the neonatal (newborn baby) unit. Baby Aria sadly died when she was two days old.

Content ArticleSafety voice is theorised as an important factor for mitigating accidents, but behavioural research during actual hazards has been scant. Research indicates power distance and poor listening to safety concerns (safety listening) suppresses safety voice. Yet, despite fruitful hypotheses and training programmes, data is based on imagined and simulated scenarios and it remains unclear to what extent speaking-up poses a genuine problem for safety management, how negative responses shape the behaviour, or how this can be explained by power distance. Moreover, this means it remains unclear how the concept of safety voice is relevant for understanding accidents. To address this, 172 Cockpit Voice Recorder transcripts of historic aviation accidents were identified, integrated into a novel dataset , coded in terms of safety voice and safety listening and triangulated with Hofstede’s power distance. Results revealed that flight crew spoke-up in all but two accidents, provided the first direct evidence that power distance and safety listening explain variation in safety voice during accidents, and indicated partial effectiveness of CRM training programmes because safety voice and safety listening changed over the course of history, but only for low power distance environments. Thus, findings imply that accidents cannot be assumed to emerge from a lack of safety voice, or that the behaviour is sufficient for avoiding harm, and indicate a need for improving interventions across environments. Findings underscore that the literature should be grounded in real accidents and make safety voice more effective through improving ‘safety listening’.
