    Medical records contain a plethora of information, from a patient’s diagnoses and treatments to marital status to drinking and exercise habits. They also note whether a patient has followed medical advice. A health provider may add a line stating that the patient is “noncompliant” or “non-adherent,” signalling that the patient has been uncooperative and may exhibit problematic behaviours. Two large new studies in the US found that such terms, while not commonly used, are much more likely to appear in the medical records of Black patients than in those of other races. The first study, published in Health Affairs, found that Black patients were two and a half times as likely as white patients to have at least one negative descriptive term used in their electronic health record. About 8% of all patients had one or more derogatory terms in their charts, the study found. The most common negative descriptive terms used in the records were “refused,” “not adherent,” “not compliant” and “agitated.” The second study, published in JAMA Network Open, analysed the electronic health records of nearly 30,000 patients at a large urban academic medical centre between January and December 2018. The study looked for what researchers called “stigmatising language,” comparing the negative terms used to describe patients of different racial and ethnic backgrounds as well as those with three chronic diseases: diabetes, substance use disorders and chronic pain. Overall, 2.5% of the notes contained terms like “nonadherence,” “noncompliance,” “failed” or “failure,” “refuses” or “refused,” and, on occasion, “combative” or “argumentative.” But while 2.6% of medical notes on white patients contained such terms, they were present in 3.15% of notes about Black patients. Looking at some 8,700 notes about patients with diabetes, 6,100 notes about patients with substance use disorder and 5,100 notes about those with chronic pain, the researchers found that patients with diabetes — most of whom had type 2 diabetes, which is often associated with excess weight and called a “lifestyle” disease — were the most likely to be described in negative ways. Nearly 7% of patients with diabetes were said to be noncompliant with a treatment regimen, or to have “uncontrolled” disease, or to have “failed.” The labels have consequences, warns Dr. Schillinger, who directs the Center for Vulnerable Populations at San Francisco General Hospital and Trauma Center. “Patients whose physicians tend to judge, blame or vilify them are much less likely to have trust in their doctors, and in the medical system overall,” Dr. Schillinger said. “Having health care providers who are trustworthy — who earn their patients’ trust by not judging them unfairly — is critical to ensuring optimal health and eliminating health disparities.” Read full story (paywalled) Source: The New York Times, 20 February 2022

    The Collaboration for Change is a group of two UK universities, nine community organisations and two small and medium size enterprises, who have conducted research on how to improve vaccine uptake among ethnic minority groups. This report outlines the results of a co-produced research study by the Collaboration for Change that combines research, evidence and lived experience to understand and improve vaccine uptake in ethnic minority communities. The report highlights the following factors influencing vaccine uptake: Using trusted messengers to encourage vaccine uptake may be a useful strategy to increase uptake in communities with low vaccination rates A lack of trust in the organisations and individuals encouraging vaccine uptake impacts vaccine uptake across many ethnic minority communities People are less likely to accept the offer of vaccination if little culturally and linguistically appropriate information is available to them, especially when their concerns are not covered Alongside the report, the Collaboration for Change has produced some recommended strategies to improve vaccine confidence and uptake.

    The purpose of this investigation by the Healthcare Safety Investigation Branch (HSIB) was to help improve patient safety in relation to the instructions 999 call handlers give to women and pregnant people who are waiting for an ambulance because of an emergency during their pregnancy. The HSIB investigation reviewed the case of Amy, who was 39 weeks and 4 days pregnant with her first child. She contacted 999 after experiencing abdominal cramps and bleeding. While waiting for an ambulance to arrive, Amy received pre-arrival instructions which were generated through a clinical decision support system (CDSS) from a non-clinical call handler. Amy was then taken by ambulance to hospital where her baby, Benjamin, was delivered by emergency caesarean section. Amy had excessive blood loss due to a placental abruption and was admitted to the high dependency unit for 12 hours following the birth. Benjamin required resuscitation to help him breathe on his own, he was intubated, and he received 72 hours of therapeutic cooling. He spent 13 days in hospital.

    The US Institute for Safe Medication Practices (ISMP) list of error-prone abbreviations, symbols, and dose designations contains abbreviations, symbols, and dose designations which have been reported through the ISMP National Medication Errors Reporting Program (ISMP MERP) and have been misinterpreted and involved in harmful or potentially harmful medication errors. These abbreviations, symbols, and dose designations should NEVER be used when communicating medical information verbally, electronically, and/or in handwritten applications. This includes internal communications; verbal, handwritten, or electronic prescriptions; handwritten and computer-generated medication labels; drug storage bin labels; medication administration records; and screens associated with pharmacy and prescriber computer order entry systems, automated dispensing cabinets, smart infusion pumps, and other medication-related technologies.

    In this blog for National Voices, Sue Brown, CEO of the Arthritis and Musculoskeletal Alliance, argues that using the word 'elective' when referring to elective surgery is misleading, and downplays the seriousness of waiting for a long time for treatment or surgery. She looks at the impact of waiting too long for surgery such as joint replacement on the lives of patients. Intense, long term pain and loss of mobility can lead to deteriorating mental health, isolation from friends and family and job loss, among other things. Patients needs support while they wait for surgery, and Sue outlines what she believes is needed to support patients who have had community and secondary care delayed: Design support with those with lived experience – ask what is important to them. Use the things we know can help, like social prescribing and health coaching – individual or group personal support. Use the voluntary and community sector who have a wealth of experience in supporting long term condition management – people need to know they are not alone and get support from others in the same situation.

    On the 15 May 2020, John Skinner was admitted to Watford Hospltal suffering from a tonic clonlc seizure. He had a background of cannabis usage and a subdural empyema in 2020 that had left him with epilepsy. On arrival at hospital he again had another tonic clonlc seizure and focal seizures. The Junior doctor Instructed to administer the drug sought advice from a more senior doctor as to the dose to be administered. As a result of a failure In verbal communication between the doctors, aggravated as both were masked, a dose of 15 mg/kg was heard as 50 mg/kg and an overdose was administered. He was given 3600 mg of phenytoln. He arrested within 16 minutes and died and could not be revived.

    Masks worn by doctors "aggravated" a miscommunication over the dose of an anti-epileptic drug that resulted in a man's death, a coroner has warned. John Skinner died at Watford General Hospital in May 2020. A coroner has written a Prevention of Future Deaths Report (PFDR) saying he feared the same could happen at other hospitals if action was not taken. Assistant Coroner for Hertfordshire, Graham Danbury, said in the report: "As a result of failure in verbal communication between the doctors, aggravated as both were masked, a dose of 15mg/kg was heard as 50mg/kg and an overdose was administered." Mr Danbury, writing to NHS England, said: "This is a readily foreseeable confusion which could apply in any hospital and could be avoided by use of clearer and less confusable means of communication and expression of number." A spokesperson for West Hertfordshire Hospitals NHS Trust said: "A comprehensive action plan is in place to ensure that lessons are learned from this incident."     In this blog for Refinery29, Sadhbh O'Sullivan looks at the issues faced during antenatal care by pregnant women who are overweight. She recounts the perspectives of several pregnant women who felt dehumanised and blamed for their weight during pregnancy. She also highlights issues with the way in which risks are communicated to pregnant women, with overcommunication and overestimation of risk causing anxiety and sometimes making women reluctant to engage with maternity services. She also discusses failures of informed consent, the role of comorbidities and the impact of wider health inequalities.

    From the perspective of a service user, interactions with health and social care are often exceedingly difficult to navigate. The NHS’s traditional to approach to managing patient pathways has involved letters, appointments at set times, and stress for an individual needing to communicate that a planned consultation is no longer needed – or is needed more urgently. Knowing which service to access, and how to do so swiftly, can be particularly challenging. All this is inefficient and can lead to poor patient experience. As the service seeks to manage the backlog of care, and to meet the continuing demands of an unpredictable pandemic, that becomes particularly problematic. So how might healthcare organisations help move from patients who are passive participants in pathways to active partners, able to regularly communicate as their needs change? How might self-referrals and patient initiated follow up processes be more widely rolled out? What unpinning technology would be needed to make such a shift?     In this editorial in the Journal of Health Services Research & Policy, Professor Brendan McCormack, Associate Director of the Centre for Person-centred Practice Research at Queen Margaret University Edinburgh, looks at the role of person-centred care in improving quality in health systems. He argues that there is a need to demonstrate the value of person-centred cultures and the significance of person-centred outcomes to healthcare organisations. In order to achieve this, researchers need to utilise theory-driven and mixed-methodology evaluation designs that demonstrate effectiveness and capture the diversity of experiences among all stakeholders.

    This scoping review in JMIR Human Factors looked at existing research into how including the reason for use on a prescription impacts pharmacists. It suggests that including the reason for use on a prescription can help the pharmacist catch more errors, reduce the need to contact prescribers, support patient counselling, impact communication and improve patient safety. Concerns about workflow and patient privacy may be factors that prevent the inclusion of use information. The review identified that more research is needed to better understand how the inclusion of use information affects pharmacists.

    This report by The Patients Association is based on information gathered from more than 1,000 patients in a survey carried out in December 2021, just before the omicron wave of the Covid-19 pandemic hit the UK. The results of the survey highlight that patients found it hard to access care during this period, with pressures affecting the NHS compromising their care. They also show that the worst affected patients were those whose illness or care needs seriously affect their day-to-day lives.

    Voices for Improvement: Facilitating the co-existence of Lived and Learned Experience Rachel Matthews, Head of Experience at National Voices and Associate Lived Experience Leader Keymn Whervin have co-designed and developed a prototype programme of coaching and mentoring, which will bring mutual benefit to those with Lived and Learned Experience and improve health and social care. We know that people with Lived Experience bring important insight to the design and delivery of care, and understand how resources can be directed for better value. However, they do not often get the opportunity to work alongside system leaders to support improvement. Voices for Improvement, a National Voices project in partnership with Q Community, offers Lived Experience Partners the opportunity to provide coaching and mentoring to those in senior positions in health and care. Through 1 to 1 coaching in action learning groups, Lived Experience Partner participants are trained to bring their perspective and insights to support leaders to develop their personal and organisational practice. We really believe that this has the potential to help to enact meaningful change through strategic co-production. The event This event will share our learning from this project as we prepare to launch. We want to bring you along for the journey, and allow you the chance to meet those who have contributed along the way. We are excited to share our methodology and invite participants to ask questions to see what getting involved in this work could look like for them, their organisations and their networks.     In this article for Nature, Aris Katzourakis, Professor of Evolution and Genomics at the University of Oxford, highlights misconceptions around the word 'endemic' that have arisen during the Covid-19 pandemic. He defines an endemic infection as "one in which overall rates are static — not rising, not falling," and highlights that we have come to associate this with less harmful illnesses such as the common cold, when in fact it can also be applied to deadly diseases including malaria, polio and tuberculosis. He argues that the word has been misused by policymakers to indicate that the virus poses less threat and therefore no action needs to be taken. In order to tackle the ongoing threat of Covid-19, the author suggests the following four actions: Set aside lazy optimism Be realistic about the likely levels of death, disability and sickness. Targets set for reduction should consider that circulating virus risks giving rise to new variants Use the weapons we have available, globally: effective vaccines, antiviral medications, diagnostic tests and a better understanding of how to stop an airborne virus through mask wearing, distancing, and air ventilation and filtration Iinvest in vaccines that protect against a broader range of variants

    This study in JAMA Network Open sought to determine whether limited English proficiency (LEP) is associated with not reporting a history of cardiovascular disease among patients with angina symptoms. The study found that patients reporting symptoms of angina were more likely to not report having cardiovascular disease if they had limited English proficiency. This discrepancy may be because of higher rates of undiagnosed cardiovascular disease or lower awareness of such diagnoses among individuals with LEP. The authors highlight the importance of finding effective communication strategies for people with LEP in order to make prevention and treatment for cardiovascular disease more effective.

    This study in the International Journal of Nursing Studies looked at the role of primary care nurses in coaching patients in shared decision making about their treatment. It evaluated an approach to support nurses in coaching patients, which was found to have a positive impact overall. Nurses became more aware of their own attitudes and learning needs and reported more in-depth discussions with patients. However, nurses struggled to integrate the approach in routine care and highlighted the need to receive support from their practice to implement the new approach.

    Improving patient safety culture (PSC) is a significant priority for OECD countries as they work to improve healthcare quality and safety—a goal that has increased in importance as countries have faced new safety concerns connected to the COVID-19 pandemic. Findings from this OECD benchmarking work in PSC show that there is significant room for improvement.
